Police said the child, Kevion Shand, showed signs of physical injury to his head, face and jaw.
The medical examiner on Sunday ruled the death of the 4-year-old a homicide.
According to the medical examiner, the boy was beaten to death and suffered blunt impact injuries to his head causing hemorrhaging.
Investigators believe the child was dead for some time before grandmother finally called 9-1-1 to report he wasn't breathing.
The grandmother gained custody of the 4-year-old about a month ago after his mother was jailed at a Pennsylvania prison, officials said.
