Police say 50-year-old Julio Velez of Mastic died when his Saturn Sedan veered into oncoming traffic, slamming into an SUV Saturday night.
It happened shortly after 8 o'clock on County Route 58 in Riverhead.
The SUV's occupants were treated for minor injuries at Peconic Bay Medical Center.
Police say a short time before, Velez had fled the scene of two other minor crashes in which he crashed head-on into other vehicles.
There were no injuries in the previous accidents.
Investigators are looking to interview anyone who may have witnessed those accidents.
