Suffolk County police say the accident occurred around 8:45 p.m. Tuesday at an intersection in North Amityville.
One of the drivers, 87-year-old Chester Spinek of East Farmingdale, was listed in serious condition at Good Samaritan Hospital Medical Center in West Islip.
The other driver, 38-year-old Naresh Wadhwa of Bay Shore, was treated for non-life-threatening injuries.
Both vehicles were impounded for safety checks.
