BRONX (WABC) -- Police are searching for a 96-year-old woman who was last seen leaving her nursing home in KingsBridge Heights section of the Bronx.

Officials say Jessie Thompson was last seen walking out of the Jewish Home Lifecare center on Sunday with all of her belongings. She had been staying at the nursing home since December.

A spokeswoman says certain residents are allowed to leave depending on their clinical status.

Officials are worried about Thompson because she has no local family and has been diagnosed with dementia.

She is 5'4 120 lbs and was last seen wearing a black jacket, brown pants and a black hat with white flowers.

A surveillance camera captured her just before she convinced a guard at the facility in Kingsbridge that she was a visitor.

No one has any idea where she might have gone.

Thompson has been at the nursing home since December, transferred from a hospital.

Before that she lived alone. As a patient with dementia, Thompson was outfitted with what's called a wander guard, a bracelet that triggers an alarm should she wander past a certain point.

It's unclear whether she removed the device or if the alarm malfunctioned or went unnoticed.

Investigators say Thompson had a guard escort her to a bus stop across the street.

The NYPD has plastered the neighborhood with flyers with her picture.
