The agency found that the charter bus was driving with worn and under-inflated tires when it crashed and overturned on Interstate 84 in Wawayanda, New York in September 2023, killing two people and injuring dozens of students.
The band director and a teacher were killed in the crash.
The victims were identified by authorities as Beatrice Ferarri, 77, of Farmingdale, and Gina Pellettiere, 43, of Massapequa.
The report said the left-front tire failed and the motorcoach was abruptly pulled to the left, crossed the left lane and shoulder penetrated the roadside barrier and rolled before coming to a stop on its left side.
The bus was operated by a 59-year-old driver and carried 40 high school students and 3 adult chaperones.
